Description

The Hazard/Incident/Accident Report will capture vital information from any reportable potential hazards, accidents or near misses on your job site, also the name of the form changes depending if it has been classified as a Hazard, Incident or an Accident causing injury. There is a place for both the employee involved and job supervisor to put their version of events, and also actions taken and recommendations. It also gathers information about the job site, dates etc from the job card.
